Introduction Hypertension is one of the chronic diseases that causes major deaths due to cardiovascular disease that requires patient compliance in taking medication and routinely checking blood pressure, which is also influenced by psychological factors. Psychological factors such as stress, depression, anxiety, perception of illness and family support can influence patient behavior in carrying out compliance with medication therapy. This study aims to determine the psychology and compliance with medication in hypertensive patients at Karsa Husada Batu Hospital. Method The research method used is a case study with a qualitative description method using research instruments, namely psychological measuring instruments, structured interviews, and blood pressure observations on 2 subjects for 14 days. Result This showed that both patients did not experience depression or anxiety, although one patient experienced mild stress. Both showed a positive perception of the disease and received good family support, and had high compliance in taking antihypertensive drugs. Blood pressure observations showed a gradual and significant decrease in patients with more stable psychological conditions. Conclusion This study shows that psychological factors, compliance with medication, and family support are important in the management of hypertension. Patients with stable and positive psychological conditions tend to have a significant decrease in blood pressure. High compliance with medication, supported by family, strengthens the effectiveness of treatment.
